The MENTOR Network, a human services organization, is about helping people. We serve people with developmental disabilities, children with emotional and behavioral challenges, and people with acquired brain injury by offering community-based, residential, in-home and out-of-home services to fit each individual’s needs and preferences.




With locations throughout southern California, Cole Vocational Services, First Step Independent Living and Unlimited Quest provide day habilitation and employment services to adults with developmental disabilities in both center-based and community-based programs.




We are currently hiring for DIRECT SUPPORT PROFESSIONALS (CAREGIVERS) for our Cole Vocational Services Day Program located in Covina, CA.




Full-Time and On-Call Positions are available.




Job Duties and Responsibilities:


• Assists individual consumers in accordance with an Individual Plan that is established for each consumer for the purpose of developing and improving a consumer’s basic life skills.


• Fosters self-reliance and independent living for individuals receiving services.


• Establishes a therapeutic relationship with the individual receiving services to ensure effective guidance, support and service delivery.


• Provides training, assistance and/or supervision to individuals in the areas of daily living, personal care, financial management, community and social integration, medication administration, and/or job skills development.


• Provides support to the individual to ensure objectives are met in accordance with the Service Plan.


• May coordinate, organize and/or assist with household activities such as light housekeeping and meal preparation according to state requirements.


• Provides transportation for individuals receiving services to planned and/or necessary activities and appointments.


• Maintains current progress and service notes and any other appropriate documentation in accordance with The MENTOR Network policy, program and Area Authority standards, and State Division of Mental Health/Developmental Disability/Substance Abuse policies.


• Completes and submits all required documentation to direct supervisor in accordance with deadlines determined by the Program Manager.


• Provides direct supervisor with verbal feedback and data relevant to the individual’s services, Service Plan or administrative policy.


• Provides emergency coverage and/or care for individuals receiving services.


• Reports incidents of maltreatment in accordance with Mentor policy.


• Complies with federal, state, and local licensing rules and regulations.


• Promotes positive relationships with consumers, families, payors, and community representatives.


• Maintains confidentiality, respects human rights and practices universal precautions in accordance with The MENTOR Network policy.




Requirements:


• High school diploma or equivalent.


• Six months to one year of experience in the human services industry preferred.


• Valid driver's license in good standing


• Car registration and vehicle insurance


• Must be able to pass health screening, TB test, and criminal background check
